CALIFORNIA COVID-19 NOTICE
I understand that in accordance with local policy, all attendees age 12 and older are required to show proof of full vaccination for COVID-19 in order to attend an event at The Warfield, and masks will be required indoors. Guests under 12 are required to show a negative test taken within 72 hours of the event. "Full Vaccination" means two weeks after your final dose of a COVID-19 vaccine. I understand that while at the event I must follow the California Department of Public Health’s Guidance for the Use of Face Coverings, available at https://www.cdph.ca.gov/Programs/CID/DCDC/Pages/COVID-19/guidance-for-face-coverings.aspx.
WARFIELD HEALTH & SAFETY POLICY
In accordance with San Francisco’s public health mandate, all attendees age 12 and older are required to show proof of full vaccination for COVID-19 in order to attend events at The Warfield, and masks will be required indoors. Guests under 12 are required to show a negative test taken within 72 hours of the event. Arrive early to allow time for vaccine proof verification checks at the door and to catch opening sets.
To be considered fully vaccinated you will need to have received your final vaccine dose two weeks(14 days) prior to the event you are planning on attending. If you have not received your final dose within this time frame, you will not be able to enter the venue. A negative test will not be accepted in lieu of proof of vaccination.
Review the San Francisco public health mandate here: https://sf.gov/information/vaccine-required
The vaccination policy will be implemented as an open-ended one, with any changes or reversals informed by updates relating to infection rates, transmission data, variant developments, and local and federal regulations.
WHAT IS CONSIDERED “FULLY VACCINATED”?
“Full vaccination” means 14 days or more following the final dose of a US FDA approved or WHO recognized vaccine against COVID-19 (e.g., Moderna, Pfizer, Johnson & Johnson, AstraZeneca/SK Bioscience, Serum Institute of India/COVISHIELD and Vaxzevria, Sinopharm, and Sinovac).
HOW DO I SHOW PROOF OF VACCINATION WHEN I ARRIVE?
Acceptable documentation may be a physical copy of a COVID-19 Vaccination Record Card, a digital copy of such card or such other proof as is permitted locally. A photo of the front and back of your vaccination card will be accepted.
WHAT IF I FORGET MY PROOF OF VACCINATION?
You won’t get in. So please don’t.
I HAVE NO INTENTION OF GETTING VACCINATED, SO WHAT NOW?
We wish you would reconsider your choice because this is a matter of public health, and we won’t get to have nice things like normal concerts without masks unless everyone gets vaccinated.
WHAT ABOUT ATTENDEES UNDER THE AGE OF 12?
As the vaccine has not been approved for use on kids under age 12, a proof of a negative COVID test (molecular, PCR or antigen) that was administered by an official testing center will be required. Test results must be dated within 72 hours of the event.
I’M VISITING FROM ANOTHER COUNTRY, WHERE OTHER COVID VACCINES ARE APPROVED FOR USE (ASTRAZENECA, FOR EXAMPLE), WILL I BE ABLE TO GET IN?
Yes, as long as you’re considered fully vaccinated.
WILL I STILL HAVE TO WEAR A MASK?
At this time masks are required to be worn while indoors by the City Of San Francisco. More information on the local mandate is available here: https://sf.gov/information/vaccine-required
WHAT IF I HAVE A MEDICAL CONDITION OR RELIGIOUS BELIEF THAT EXEMPTS ME FROM GETTING VACCINATED?
Any patron needing an exemption due to a medical condition, disability or religious belief will need to complete the Patron Vaccination Exemption Form. Bring the signed version along with a proof of a negative COVID test (molecular, PCR or antigen) that was administered by an official testing center to the event. Test results must be dated within 72 hours of the event. Home testing kit results will not be accepted for entry.

COVID-19 is an extremely contagious disease that can lead to severe illness and death. There is an inherent and elevated risk of exposure to COVID-19 in any public place or place where people are present and there is no guarantee, express or implied, that those attending a Goldenvoice event will not be exposed to COVID-19.
ATTENDEE PROMISE & HEALTH ACKNOWLEDGEMENT
All attendees agree to follow venue policies (including health and safety policies) and posted instructions while at the venue. According to the CDC, older adults and people of all ages with serious underlying medical conditions may be at higher risk of death or severe illness from COVID-19. All attendees should evaluate their risk in determining whether to attend the event. By entering the venue, attendees voluntarily assume all risks related to exposure to COVID-19 and confirm that they will adhere to local quarantine mandates and the CDC quarantine requirements, available at https://www.cdc.gov/coronavirus/2019-ncov/if-you-are-sick/quarantine.html.
